A sullen EJ looks at the photo of Sami and his son before tossing it aside. Kate enters and mentions how he gave Stefano quite a scare. She apologizes for waving her gun around at him and Stefano earlier and claims she was upset and emotional. She denies being the one who put him in the hospital and takes his hand, expressing how she wishes she could do something for him. He laments following his father’s advice and taking Samantha. He claims he now knows that he can’t force Sami to love him. Kate offers to at least try to get Sami back to the hospital. EJ, though, insists Stefano’s the only one he can count on.
Tony later places the blame onto Stefano for EJ’s condition, since Stefano was so blinded by revenge. Stefano rants how it was a mistake asking him for help, noting how he’s not even a DiMera. Kate emerges from EJ’s room and tells Stefano that EJ’s wants to see him.
Stefano sits down beside EJ, who professes to love him. Stefano expresses his love as well. EJ then asks his father to bring him a gun.
At Sami's apartment, Colleen implores her to be true to herself and claims that Sami’s the only one who can get through to EJ. Sami relays how EJ told her to go back to Lucas. Colleen refers to this as a load of rubbish. “It’s you he wants,” says Colleen. Sami claims that if EJ wanted her, he wouldn’t have sent her back home to Lucas. Colleen insists she’s fooling herself and taking the easy way out, suggesting that she’s afraid of what she feels. She refers to Sami and herself as two peas in a pod. Sami tells her to go away, then turns the music up loud. Colleen still refuses to disappear. Lucas emerges from the bedroom and tells her to turn it down, since she’s likely to wake up the twins. She apologizes before Philip knocks on the door. Lucas goes outside to talk to his brother. Colleen asks Sami if she’d be able to live with herself if EJ dies. Sami folds clothes and tries to convince herself that Colleen’s just a figment of her imagination. Colleen insists that part of Sami wants to help EJ. “I don’t care about EJ,” says Sami. Colleen believes otherwise and notes that she’s still wearing his wedding ring. Sami tries to remove it, but it’s stuck. Colleen implores Sami to go to EJ and tell him she loves him. Sami balks at this idea. Colleen predicts that if Sami takes her advice and EJ lives, then Stefano will be grateful to her for the rest of his life.
Out in the hall, Lucas confronts Philip about the gun Kate gave to him and insists he needs it back. Philip refuses to hand anything over to Lucas until he spills why he needs it. He insinuates that their mother is protecting someone, like Lucas perhaps. Lucas claims he was at home the whole time when Sami walked down the aisle and doesn’t care whether or not Philip believes him. Lucas insists he can’t tell him any details, since it will only complicate things. Philip reveals that the gun is gone and claims he tossed it in the lake, which infuriates Lucas. Lucas reveals that Sami had a bullet hole in her dress, which may have meant that she was a target. “And you think Dear Old Mom did it,” assumes Philip. Lucas adamantly insists that he needs to know if someone fired the gun. Philip doesn’t provide a direct answer and insists they need to protect their mother, but doubts Lucas can do that if the DiMeras suspect her.
Sami later lies on the couch and states her refusal to lie to EJ. Colleen reminds her of the real reason she married EJ. Sami speculates that even if she did tell EJ that she loved him, he wouldn’t believe her. Colleen implores her to stop making excuses and go to EJ. She wonders if Sami’s afraid of her feelings for EJ. Sami insists that saying “I love you” isn’t a miracle cure for someone who’s dying. Sami then answers the phone to Kate, who urges her to come to the hospital. “It’s EJ. He’s dying,” she desponds.
Out in the hall, Lucas can’t make any promises, so Philip refuses to give any answers. Philip implores Lucas not to sell out their mom, despite the misdeeds she’s done in the past. Lucas says he won’t, but states he won’t lie for her either. Sami then comes outside and asks Lucas to watch the twins, claiming she has errands to run.
Lucas proceeds to go back inside and insists he wouldn’t do anything to hurt their mom. However, if she put a bullet in EJ, neither one of them can protect her. After Lucas goes inside, Philip pulls out the concealed gun from his pocket.
Back at the hospital, a despondent Stefano emerges from EJ’s room and tells Tony and Kate that EJ asked him for a gun and wants to end his life. “I will not rest until I find out who has done this. And when I do, God help them,” he vows. Tony tells him to take a good look at himself in the mirror next time he decides to go after someone. Stefano orders Tony out of his sight. Sami then arrives and wonders what the status is on EJ. Kate informs her that he has a high fever and a serious infection. Stefano asserts that he’s lost his will to live and warns Sami not to upset his son. Sami enters a weary EJ’s room and picks up the photo of herself and Johnny. She apologizes for their fight earlier. He tells her to go away and go be with Lucas, since he’s the one she loves. “What if he’s not?” she asks.
